New Always "Incorrect PIN code"

Hi,
today i updated my galaxy nexus on 4.2.2 - MIUI 3.7.5, and when my phone ask me for PIN code, i entered correctly but it always show "Incorrect PIN Code". Even when i entered more than 3 times "Incorrect PIN Code", it didn't want the PUK code, and continue to displayed the same "Incorrect PIN code". Whatever i entered, it displayed that ... Even i delete, reset ALL my data and reinstall the phone, it had the same problem.
I tried my SIM on another phone - work. I tried SIM with another mobile operator on my phone - work, but when i tried another SIM with the same mobile operator as mine - didn't work.
Can U help me to resolve this strange problem ?
If it matters, Im on VIVACOM Bulgaria(mobile operator).

to not make a new thread, i'll write here

also noticed that when I connect my phone to pc, it doesn't show main folders (music, DCIM, Android) on pc. Phone is connect as " Files" Can I fix this ?
